Endosonographic staging for N1 disease.

چکیده

I congratulate Dooms et al 1 on their important study recently published in CHEST (January 2015) that investigated the role of endosonography for mediastinal nodal staging in patients with operable and resectable lung cancer and suspicion of N1 involvement based on CT and PET imaging. To my surprise, the sensitivity of endosonography to detect N2 disease was only 38%, with a negative predictive value of 81%. First, the data show that the sensitivity of endosonography in subcentimeter fl uorodeoxyglucose-PET nonavid nodes is inferior in comparison with the sensitivity of enlarged and/or fl uorodeoxyglucose-PET avid nodes (94%). 2 Second, of key interest are the 14 patients whose conditions were staged as false-negative. In only one of these patients, endobronchial ultrasound (EBUS) was followed by endoscopic ultrasound (EUS) or EUS performed with the EBUS scope (EUS-B). Importantly, in eight of the 14 patients, the missed metastases (located in stations 4L, 7, and 8) were located well within reach of EUS-B, and in two more patients, nodes in station 5 could have been well visualized and possibly sampled. In this scenario, as stated by the authors, a sensitivity of . 70% theoretically could have been reached.

Prognostic analysis of esophageal cancer in elderly patients: metastatic lymph node ratio versus 2010 AJCC classification by lymph nodes

BACKGROUND Recent studies have proposed a new prognostic factor (metastatic lymph node ratio, or MLNR) for patients with esophageal cancer (EC). However, to the best of our knowledge, there have been no studies conducted to date regarding MLNR in elderly patients. The aim of this study was to determine the prognostic value of MLNR staging compared with the 2010 American Joint Committee on Cance...

Endosonographic Staging of Esophageal Cancer.

BACKGROUND: Endosonography (ES) is an important tool for staging malignant esophageal cancer with the TNM staging classification. ES is a safe procedure and an accurate method of staging tumor invasion (T) and lymph node involvement (N). METHODS: The author reviewed the literature on the comparative roles of computed tomography and ES as complementary staging procedures. RESULTS: Advantages of ...

The role of segmental nodes in the pathological staging of non-small cell lung cancer

BACKGROUND Segmental nodes are not examined routinely in current clinical practice for lung cancer, the role of segmental nodes in pathological staging of non-small cell lung cancer after radical resection was investigated. METHODS A total of 113 consecutive non-small cell lung cancer patients who underwent radical resection between June 2009 and December 2011 were retrospectively reviewed. A...

The IASLC Lung Cancer Staging Project: "the nodal zone".

The paper by Rusch et al.1 represents a Herculean effort by the staging committee of the International Association for the Study of Lung Cancer (IASLC) to determine whether the current nodal descriptors for non-small cell lung cancer2 should be maintained or revised. A retrospective international lung cancer database was developed with staging and outcome data on 100,869 lung cancer cases manag...

Stage IV prostate cancer: survival differences in clinical T4, nodal and metastatic disease.

PURPOSE In the prostate specific antigen era most prostate cancer presents at an early stage. However, a significant number of patients have advanced disease, including those with stage IV disease. Assignment to stage IV prostate cancer may occur by different modes, namely as T4N0M0 vs N1 vs M1 disease. We hypothesize that patients with clinical T4 disease have better outcomes than those with N...
